Is there a documentation guideline regarding giving this vaccination by a RN to the patient? The pediatrician that asked this question wants to know if the vaccine sticker label with the RN signature and date it is enough documenation? My thoughts are that every vaccine given should be documented on the patient’s vaccine record with lot number, brand, location of shot, and RN signature... also the initials of the parent or guardian who authorized the shot.

Any suggestions?

And federal law requires that you give the patient the vaccine information sheet, which you also can print out from the CDC website.
